Definition: Migraine

The simplest, although not completely accurate definition of migraine is an intense, throbbing, single sided headache of a reoccurring nature. It is a common misconception that migraine is just another word for “really severe headache.” The truth is, migraines do not always cause severe headaches – in fact, sometimes you will not have a headache at all! Severe headaches are not always migraines either. Tension headaches can cause severe headaches as can secondary headaches – pain caused by another medical condition such as hemorrhage.

Instead, it is easiest to think of migraine as a chronic neurological condition. First lets address the neurological portion of the definition. Migraine is basically an umbrella term for a variety of symptoms that share a common neurological cause. At this time it is believed that migraine attacks are caused by neuron excitability in the brain, which leads to the inflammation and swelling of blood vessels in the brain. This irritation then starts to affect other portions of the brain which cause perceptions of pain, visual disturbances, nausea, dizziness, numbness, etc. Second, what do I mean by chronic? Well, having migraines is a condition you can manage but not cure. There is no magic solution. You can figure out what is causing your migraines and do your best to avoid these triggers, but if exposed you will likely develop a migraine. You may also find a medication or combination of medications (or other type solution) that works to reduce or eliminate your migraines – but in order for it to work you must take the medication, otherwise your migraines will return.

Another commonly misunderstood fact is that migraines do not have a precise set of symptoms that everyone will experience. Once you realize that the symptoms of migraines are caused by the inflammation of blood vessels in the brain, it is not that much of a stretch of imagination to understand that many people’s symptoms (or perception of symptoms) will be unique.

When it comes to defining migraine symptoms, here is a common list but keep in mind there will be variations:

  • A throbbing, or trickling type pain on one or both sides of the head.
  • Pain levels range from moderate to unbearable.
  • Temporary symptoms of visual disturbances (blurry vision, double visions, seeing flashing lights,) strange tingling or numb sensations in one’s limbs.
  • Sensitivity to light or sound
  • Nausea
  • Lack of appetite
  • Strange cravings
  • Exhaustion
  • Unexplained mood swings

Even if you have not experienced all the symptoms listed above, it does not mean that you are not experiencing a migraine. Track your symptoms and speak to a doctor!

Exertion Headache

Have you ever been exercising and then experienced a headache? For some people physical activity can trigger and headache that can develop into a migraine (called exertion migraines.) Exercise induced headaches are not really that uncommon and in most cases are completely harmless – although uncomfortable.

What Causes an Exertion Headache

Most commonly exertion headaches develop after a long bout of exercise or after a very intense burst of physical activity. Specific activities include running, heavy lifting, rowing, swimming, tennis, etc.

Among most sufferers, this condition is most commonly seen among men in their 20’s and those with a family history of exertion headaches.

The exact reason that people experience exercise induced headaches is not known. Researchers theorize that exercise could cause the blood vessels inside the brain to swell – causing pain and irritation.

Symptoms of Exertion Headache

Headache or throbbing migraine that occurs right after prolonged or vigorous exercise.

Most people report pain on both sides of the head.

Exertion headaches can last anywhere from a few minutes to a couple days.

Remember, if you experience any sudden, unusually severe headache (or your headache is accompanied by vision problems, nausea, or a stiff neck) please get this condition examined by a doctor. While very rare, sometimes exertion headaches are symptoms of other serious conditions (such as a tumor, brain hemorrhage or aneurysm.) At this time your doctor could suggest diagnostic test such as an MRI just to make sure nothing is seriously wrong.

Preventing Exertion Headache

If you know that you have a tendency towards developing exercise induced headaches, it could be useful to know that this condition is usually made worse by hot weather, humidity, and high altitude. Some people choose to avoid activities that trigger headaches. This is a personal decision – just be aware that there treatments available should you not want to discontinue favorite activities!

The most common preventative drugs are indomethacin (anti-inflammatory)  and propranolol (blood pressure medication.)

Indomethacin is a nonsteriodal anti-inflammatory drug commonly used to treat pain and inflammation caused by conditions such as arthritis. Indomethacin works by reducing pain and inflammation causing hormones called prostaglandins.

Side effects of indomethacin include:

  • Increased risk of heart attack or stroke.
  • Gastrointestinal holes and bleeding (risk increased when alcohol is consumed.)

Propranolol is a beta-blocker, which is frequently prescribed to treat hypertension (high blood pressure.)

Both drugs are Pregnancy class C, meaning the medication could be harmful to a developing baby. Likewise, it is not considered safe to breastfeed while taking these medications as the medication can pass through into breast milk.

Depending on the frequency and predictability of your headaches as well as your daily activities, your doctor could recommend taking medication anywhere from a few hours before activity to every single day.

Abdominal Migraine

As many as one in twenty people experience a headache nearly every single day.

Fact #2: Abdominal migraines are experienced by children and adults

An abdominal migraine is seen primarily among children and can be described as uncomfortable stomach pain with no obvious cause (ie. not food sickness, the flu, appendicitis, ulcer, etc.) This condition is also related to  “cyclic vomiting syndrome.” Cyclic vomiting syndrome tends to result in much more severe vomiting -  children commonly have about twelve episodes per year, which can last a day or two, and result in vomiting as frequently as twelve times per hour! Adults are less commonly affected, having maybe four occurrences per year but each episode can last as long as a week. Typically these attacks come on either late at night or first thing in the morning.

Symptoms of Abdominal Migraines

Typically pain in the stomach accompanied by nausea, vomiting, and gagging.

Additional symptoms can include light sensitivity, cold hands, dizziness, and headache.

Symptoms such as high fever, stiff neck, weakness, slurred speech and confusion are serious and could indicate a condition other than abdominal migraine. Make sure you or your child receives medical examination right away.

Remember – any condition in a child that seems abnormal should be a cause for concern. Please make sure that serious conditions and disorders (such as appendicitis) have been ruled out by your doctor before you explore other treatment alternatives – especially if the migraines are becoming more frequent and severe.

Fact #3: Abdominal migraines have the same root cause as migraines!

These types of migraines are most common among children who have a family history of migraines, meaning one of more close relatives also suffer from migraines. Usually upon reaching adulthood, one’s abdominal migraines usually do not continue but rather develop into migraine headaches.

  • Strong odors
  • Cleaning chemicals
  • Changes in weather
  • Dust (very common problem. Be sure to wash all pillows, blankets, and stuffed animals.)
  • Flickering lights

If you suspect environmental triggers are a problem, consider increasing your cleaning schedule being especially careful to get all surfaces – including under the bed. There are also certain mattress pads and pillow covers that will block dust mites. Don’t forget that the feathers in a feather pillow can also be an allergen.

Treating Abdominal Migraines

Prescription migraine medications such as tricyclic antidepressants and triptans are frequently helpful at preventing abdominal migraines.

During an attack:

Prevent dehydration as vomiting can cause rapid water loss.

Stick to a easy to digest diet and avoid foods high in spice and fats. Some people also do well avoiding citrus fruits and juices.

Get plenty of sleep.

High Blood Pressure Headaches

Many people worry when they start to have regular headaches, if high blood pressure could be the cause. Most of the time this is not the case. Usually people who have high blood pressure do not experience any warning symptoms. For this reason high blood pressure is referred to as “the silent killer.” Sometimes headaches, dizziness, blurred vision, and/or nausea can be symptoms of this condition.

How do you know if you have high blood pressure?

Its simple, all you need to do is get your blood pressure checked by your doctor. High blood pressure would be considered anything higher than 140/90.

High blood pressure can be managed through medication and also reduced by minimizing one’s risk factors such as smoking, excessive alcohol consumption, weight gain, stress, and increasing physical activity.

Fact #1: Doctors Want to Find Physical Evidence of Your Pain in the Back of the Head

This is not inherently a bad thing. Anytime you experience unusual or ongoing pain you should visit the doctor to rule out other serious problems – especially if you have recently (within the last year and a half) been injured or in an accident of any kind. When you experience pain in the back of the head or neck it is always possible the source of your pain is a structural problem or misalignment which can be diagnosed by xrays and other types of medical imaging.

There are many physical causes of pain or headaches in the back of the head or neck including:

Cervicogenic Headaches, which are caused by problems with the top two or three levels of the cervical spine.

Cranial or Occipital neuralgia, a source of nerve pain which can cause ongoing pain (of an “electroshock-type” sensation) in the back of the head, neck or shoulders.

When xrayed, lots and lots of people will show signs of spinal misalignment, structural deviations, arthritis, etc – and yet, the vast majority will be completely unaware as they will not be experiencing any pain. As much as we would like to logically pair physical conditions with pain, these two do not necessarily go together for many people. Sometimes poor posture or a poorly designed work space can cause pain in the back of your head, neck, and shoulders. This type of posture is known as “head forward posture.” Head forward posture can be identified by looking at one’s posture from a profile angle (from the side.) If at all possible, have someone take a picture of you from the side. This would work much better than trying to analyze one’s own posture in the mirror.

Look at how you stand. Is your head a straight line up from your neck or does it appear to angle forward? Ears should be directly in line with one’s shoulder. If instead your head appears to jut forward when viewed from the side, poor posture could be causing some of the pain in the back of your head and neck.

Why is this? Forward head posture places too much stress on one’s muscles at the base of the skull (the suboccipitals) and the muscles in the back of one’s neck (cervical extensors.) This kind of pain can manifest as stiffness from the back of your shoulders to the top of your head and can lead to a headache located in the back of your head.

Here is another test – do you have full range of motion in your neck? Can you look all the way to each side? Can you allow your head to fall forward until your chin hits your chest? Without pain?

So what can you do if you suspect this is your problem? Essentially you will need to stretch the tight muscles and strengthen the weak ones. The most effective exercises are the following:
